This survey is being conducted on behalf of Berryfields Medical Centre and Meadowcroft Surgery.

Berryfields Medical Centre and Meadowcroft Surgery are asking patients to share their views on a planned merger between the two practices.

As we are sure you are aware, in recent years there have been increased pressures placed upon General Practice. The increasing elderly population, the growth in long term illnesses like diabetes and asthma and the need to move more work from hospitals into the community means that General Practice needs to work in different ways to absorb this increased workload. We feel that the best way to continue to offer our patients an innovative, sustainable, high quality service into the future is to merge our two practices.

We plan for this to take effect from 1 April 2022, when the practice will change its name to Berrycroft Community Health Centre (BCHC)

By merging we will strengthen our ability to maintain high quality care for our patients and invest in innovation – for example, to improve the ease and speed of patient access to healthcare, and to improve our collaboration with allied community groups. It will also allow us to create a stable and sustainable GP Practice by sharing the resources and expertise.

Berryfields Medical Centre and Meadowcroft Surgery will both remain open after the merger, until our new site at Martin Dalby Way is ready (you can find out more about this new site on the Berryfields and Meadowcroft practice websites). Then the existing buildings will close and staff will relocate to the new site. We expect this to be sometime before the end of summer 2022.

There will be very little change to patients to begin with – patients will continue to see the same staff in the same way until the new building opens. We expect the merged practice to have a new name and will update you when this is confirmed.

We would like to make sure we listen to and understand any feedback or concerns our patients may have about the merger between our practices. To share your views, please take part in this short survey.

Thank you for taking the time to give us your feedback.
