Your experience as a young person with SEND

Overview

Please complete this survey if you are between 12 and 25 years old and have a special educational need or disability (SEND).

In this survey we ask about the things you do to keep active. If you are 16 or older we will also ask you about getting ready for being an adult.

It will take about 5 or 10 minutes but don’t worry if you finish it quicker or take a bit longer than that.

What we will do with your answers

We will use the answers to see if we can improve the services and activities available for young people with SEND. We will only use your answers to help us improve things.
